The below form is for ACHS accreditation and Clinical Indicator Program members to gain access to the secure section of the ACHS website.
Members requiring access to online databases ART, EAT or PIRT, can also gain access to these tools by ticking the appropriate boxes and filling in additional fields in the form below. The initial approval for ART/EAT access needs to be provided at the organisation level by the nominated ACHS Primary Contact. Remember to include the organisation code you require access for. For access for more than one organisation, please fill in the form once for every organisation.
